What about a birth certificate and social security card?
The Midwife will complete the Birth Certificate application which also requests the social security card if you desire. You will automatically receive the social security card in the mail within 4-8 weeks. In order to receive a copy of the birth certificate you will need to contact the local Vital Statistics office, or by mail through the State Vital Statistics Office and pay a fee.
